Documentation and reporting of assessment results

4.5 All assessment results should be documented and reported in an accessible format to people with aphasia and their families.

Rationale: 
As with the results of goal setting discussions, the results of assessments should be presented in written, aphasia-friendly formats, so that clients and their families can take results away with them, learn from them, and keep them as part of a record of change over time. Clients should be encouraged to keep all their aphasia assessment results in one folder so that transferring to new speech pathologists results in a smooth transition. Having an accessible record of assessment results contributes to the idea that assessment can itself be a learning opportunity. This sharing of information can also equalise the relationship between the client and the speech pathologist, and increase the transparency of the rehabilitation process.
